ALL ATTENDEES AND FACILITATORS MUST REGISTER IN ADVANCE

NOTE: If you are registering for the Dropout Summit through the State Farm website and you are NOT from Illinois, please select Area I and region 14 when asked which area you are from. Please fill out all other information including your real address correctly.

Also, if you are guest of birth2work or a facilitator for the event, please mention it in the notes section of the registration form.
